Feasibility report on ozone therapy for pain

1、 Project background and significance:

In 2000, the World Health Organization officially proposed that "chronic pain is a type of disease".

In 2004, the International Association for the Study of Pain designated October 11th as World Pain Relief Day.

In July 2007, the Ministry of Health of China officially listed pain management as a first level diagnosis and treatment subject.

In 2008, the Chinese Academy of Sciences and the Pain Society of the Chinese Medical Association ranked pain as the fifth major vital sign after respiration, pulse, blood pressure and body temperature.

In 2009, the Pain Branch of the Chinese Medical Association organized the compilation of the "Guidelines for Clinical Pain Diagnosis and Treatment" and the "Technical Standards for Clinical Pain Diagnosis and Treatment". Pain therapy has been included in the name of the Chinese Medical Association's medical accident technical appraisal expert group.

It can be seen that the issue of pain is increasingly receiving widespread attention from the country and the whole society. Up to now, pain disease has become a disease with high incidence rate and wide distribution that endangers human health. Although it does not pose a direct threat to life like cardio cerebrovascular disease, cancer and other diseases with high mortality, it poses a great burden on people's life, work and family. Once suffering from such diseases, the quality of life will inevitably decline, and the dignity of life will not be guaranteed.

In the past two decades, various new therapies and technologies have emerged to overcome the torment of pain and illness on patients, such as the application of low addictive neurological drugs, nerve block (interruption), various minimally invasive surgeries, minimally invasive needle techniques, including traditional Chinese medicine treatment methods, and so on; The above methods have also achieved certain results, but from a clinical perspective, they also have their own inherent shortcomings. Pain remains an insurmountable barrier for humanity at present.

1. Overview of Medical Ozone

Ozone is composed of three oxygen atoms and is an allotrope of oxygen. Its density under standard conditions is 2.143g/L (higher than that of air). Ozone has strong oxidizing properties second only to fluorine, with a solubility 13 times greater than oxygen and 25 times greater than air. At room temperature, the half-life is about 20-30 minutes, and it naturally decomposes after a few hours. Medical ozone has extremely strong oxidizing ability, improves microcirculation, and promotes wound healing.

2. Principle of ozone therapy for soft tissue pain:

(1) Relaxing blood vessels improves microcirculation and oxygen supply in tissues: Ozone can stimulate endothelial cells to release NO, while endogenous NO can directly cause vasodilation and inhibit platelet adhesion and aggregation on the surface of endothelial cells. NO vasodilation may be achieved through the following mechanism: NO increases the activity of adenylate cyclase, promotes the production of cGMP in cells, activates cGMP dependent protein kinases, and inhibits protein kinase C phosphorylation, leading to the dephosphorylation of myosin light chain. The intracellular Ca2 concentration decreases. The sensitivity of contractile proteins to Ca2 is weakened. The activity of K channels on the muscle cell membrane also decreases, leading to vasodilation. In addition, ozone can increase the content of 2,3-DPS in red blood cells, shift the hemoglobin oxygen dissociation curve to the right, and increase the release of oxygen. The combined effect of these two can improve local blood circulation in tissues, increase oxygen supply, and quickly produce analgesic effects.

(2) Ozone activates antioxidant enzymes and scavenges free radicals: Ozone, as a superoxide, can activate the overexpression of antioxidant enzymes, neutralize excess reactive oxidation products (superoxide dismutase - breaks down excess peroxide free radicals; catalase - breaks down hydrogen peroxide; glutathione dehydrogenase - breaks down organic oxides), and eliminate excessive free radicals and peroxides in physiological and pathological processes. Ozone can also induce the release of cytokine antagonists and immunosuppressive factors such as IL-10 and TGF - β 1. It has been proven that long-term, low concentration ozone therapy can increase the activity of antioxidant enzymes.

3. Treatment methods

Tennis elbow, heel pain syndrome, third lumbar transverse process syndrome, sacral spinal muscle attachment strain, etc. are generally injected at one point, with two layers of infiltration under and outside the periosteum; Shoulder periarthritis and femoral arthritis generally involve infiltration in three anatomical sites: the bony process site, synovium, and joint cavity; Interspinal and supraspinal ligament strain, shoulder and back fasciitis, and strain are treated with fan-shaped infiltration of pain tissue. All procedures should be confirmed not to be in the blood vessels or nerves. The general concentration of ozone therapy is 20-40ug/ml, with 4 sessions per course, with a one week interval between each course.

Medical ozone has the characteristics of safety, simplicity, and multiple effects. Ozone therapy is an emerging minimally invasive technique in recent years. Therefore, ozone therapy is an effective method for treating various pain conditions and is worthy of clinical promotion and application. The application of ozone therapy in clinical practice has just begun in China. It is believed that with the increase of research and the accumulation of clinical experience, there will be greater breakthroughs in the treatment of various types of pain. Sincerely hope to work with people from all walks of life to continuously innovate, develop, and enrich ozone therapy technology through clinical practice, so that this new technology belonging to natural therapy can benefit all patients.
